Older Adults Services Specialist
Role
The Focus Points Family Resource Center’s Family Support Services (FSS) Department helps vulnerable families connect to resources, services, and tools needed to become self-reliant in our Globeville, Elyria, and Swansea neighborhoods and other nearby communities. The Older Adults Services (OAS) Specialist builds nurturing relationships with our older adults and empowers them to identify appropriate services. From there, the OAS Specialist collaborates with older adults in identifying their strengths and needs to develop individual case management plans. Furthermore, the OAS Specialist will collaborate with the Family and Community Resources Specialist with monthly community gatherings for older adults. Additionally, the OAS Specialist will collaborate and identify additional resources and services with the individual older adult households have and provide a warm hand-off to the referral agency to ensure the participant and organization’s contact have been introduced and understand next steps. Finally, the OAS Specialist will enter data into the identified databases they will be working on, which includes and is not limited to SalesForce, monthly reporting tools, etc.

Scope of Organization:
For nearly 30 years, Focus Points has worked successfully to transform the lives of low-income and working families in Northeast Denver and beyond. Focus Points serves over 3,000 participants each year, most of whom are Spanish-speaking immigrants. Responsibilities & Activities
- Completes 30/60/90-day onboarding work plan.
- Receives training in Family Development Credentials within the first six months, (i.e., Standards of Quality, CFSA2.0, Motivational Interviewing, Family Development Pathways, Family Development Implementation Strategies, 5 Protective Factors, etc.)
- Assists older adult individuals/families with support services.
- Holds a sustainable case management load of older adult individual households per fiscal year with CFSA2.0, goal setting program.
- Completes required CFSA follow-up with participants after 30, 60 and 90 days and more often if necessary.
- Collaborates with the Family and Community Resources Specialist in hosting monthly group connections for older adults.
- Coordinates referrals for older adult participants to outside agencies or other Focus Points programs when necessary.
- Identify community partners that provide additional resources and services for older adults.
- Makes internal referrals directly with Mental Health clinician and other departments, as needed.
- Supports families with public assistance applications (i.e., LEAP, EOC, Medicaid+, SNAP, TRUA, SSI, SSDI, and others).
- Meets with Family Support Services and Community Resources team regularly to discuss activities, budget, goals and objectives, as well as relevant issues pertaining to family development services and programs.
- Participates in maintenance/revisions of yearly program calendar.
- Collaborates in developing professional goals (6 months and yearly) and provides feedback on areas of growth and celebrations.
- Participates in webinars and other professional development opportunities as required by grants or program area.
- Maintains a professional attitude and adheres to a high level of confidentiality toward participants and Focus Points staff.
- Other duties as assigned.
Required Competencies
- Bilingual and Biliterate: ability to understand and communicate effectively with English and Spanish speaking participants and staff through oral and written expression.
- Computers Skills: ability to independently work on computers, use the internet, and Microsoft Office 365 (such as Outlook, SharePoint, OneDrive, Word, Excel, and Teams); virtual calls, and database entry.
- Cultural Awareness: supports participants in a culturally responsive manner which reflects their values, needs, and personality.
- Empathy: the ability to identify with another person’s situation on an intellectual and emotional level.
- Verbal and Written Communication: ability to articulate participants’ needs and status reports to team and/or supervisor, staff in the organization, and/or network providers to gather information and make warm referrals as needed.
Preferred Competencies
- Critical Thinker and Solution-Focused: ability to collaborate with internal team when problems arise to problem-solve and provide creative solutions to challenging, multi-faceted problems.
- Data Entry: ability to use internal databases to enter and track participants supported by Family Support Services, such as Colorado Department of Human Services’ SalesForce and Family Resource Center Association’s SalesForce.
- Flexible and Adaptable: ability to adjust to changes in duties or schedule as required by program and service delivery. Weekend and/or evening events may occur.
- Interpersonal Skills: guide conversations to assess participant’s needs and goals.
- Resource Navigator: assist participants to access necessary services through direct service or referrals to partner organizations.

Reports to: Senior Manager of Family Support Services
Status: Full-Time – 2-year position with possibility of extension
Salary: $50,000 – $55,000/year
